Type 1 Diabetes Mellitus

 

Type 1 Diabetes Mellitus (T1DM) is a chronic condition in which the pancreas produces little or no insulin because the body's immune system attacks the insulin-producing beta cells. It can develop in children, teenagers, young adults, or later in life. Unlike type 2 diabetes, type 1 diabetes is not caused by lifestyle or dietary habits.

 

With appropriate insulin therapy, glucose monitoring, nutrition, physical activity, education and regular medical follow-up, people with type 1 diabetes can manage their condition and maintain an active life.

Symptoms of Type 1 Diabetes Mellitus

Symptoms may develop relatively quickly, sometimes over a few weeks or months. Common symptoms include:

  • Frequent urination

  • Excessive thirst

  • Increased hunger

  • Unexplained weight loss

  • Persistent tiredness or weakness

  • Blurred vision

  • Irritability or mood changes

  • Nausea or vomiting

Watch for Diabetic Ketoacidosis (DKA)

When there is severe insulin deficiency, the body may start producing excessive ketones, leading to diabetic ketoacidosis (DKA). Warning symptoms can include vomiting, abdominal pain, dehydration, rapid or deep breathing, fruity-smelling breath, severe weakness and confusion. DKA is a medical emergency and requires urgent treatment.

Diagnosis of Type 1 Diabetes

Diagnosis begins with blood-glucose testing. Diabetes may be diagnosed using HbA1c, fasting plasma glucose, a 2-hour oral glucose tolerance test, or random plasma glucose when classic symptoms or a hyperglycemic crisis is present.

1. Diabetes-related autoantibodies

Tests such as GAD, IA-2 and ZnT8 antibodies can help identify autoimmune type 1 diabetes.

2. C-peptide

C-peptide testing may help assess the body's remaining insulin production in selected patients.

3. Ketone testing

Blood or urine ketones may be checked, particularly when blood glucose is high or DKA is suspected.

4. Additional evaluation

The doctor may assess other autoimmune conditions and overall metabolic health depending on the individual's age, symptoms and clinical history.

Treatment of Type 1 Diabetes

Insulin is essential for people with established type 1 diabetes. Treatment aims to replace the insulin the body can no longer produce and maintain glucose levels within an individualized target range.
